Fees & Insurance
Fees
Currently, the approximate fee
for an initial office visit is $250.00 and $125.00 for a
follow-up appointment. This does not include the additional
charges that will apply for any procedures or tests done at the
same visit. Payment is due at the time of service. To facilitate
transactions, it is our policy to keep a credit/debit card
number on file to automatically charge the amount owed after
each cosmetic procedure, unless you pay by cash or check.
Insurance
Our office will let you know if
Dr. Baker is a provider with your insurance company, but it is
your responsibility to check with your insurance company
regarding your coverage under your specific insurance plan. If
Dr. Baker is a provider for your health plan, we will bill your
insurance plan directly for services provided. At the time of
service, you are, of course, responsible for co-payments and
deductibles.
Appointments
Your appointment
time is held exclusively for you. We know your time is valuable
and we make every effort to run our office “on time”. We ask
that you arrive promptly for your appointments; ten minutes
early for an initial appointment if you have not filled out
information forms prior to your arrival. If you are unable to
keep an appointment, please call the office two business days
before your scheduled appointment to cancel, or you may be
subject to an office visit charge. Insurance companies will not
cover this charge, and you will be expected to pay the entire
amount.
We will call you 72
hours in advance of your appointment. Please provide us with
the BEST contact number. If we do not reach you, but do reach
your voicemail, you must call our office back and confirm your
receipt of our message 24 hours in advance of your appointment.
If we do not hear from you, your appointment time may be
assigned to another patient waiting to be seen.
Prescription Refills
Please call your
pharmacy at least 24 hours in advance when you need a
prescription refilled. This will allow sufficient time for the
pharmacy to contact our office for authorization, if necessary.
Please don’t request a prescription refill after office hours or
on weekends, except in an emergency.
Appropriate follow-up visits, at least once a year,
are required for ongoing prescription refills.
Medicare
Dr. Baker has opted
out of Medicare. This means that by law
our office may not bill Medicare for
any services provided to Medicare beneficiaries. You,
as a beneficiary, may not bill
Medicare for any services provided by Dr. Diane R. Baker.
Medicare eligible patients must sign a
waiver prior to receiving any medical service at this office and
agree to be responsible for payment, themselves, at the time of
service.
Patient Privacy
Your health care
information is confidential and will not be disclosed to anyone
without your written consent. The only exception would be
instances in which we are required by law to disclose your
medical information without your authorization.
